One can start with audience control and reply control. These are no-brainers and that takes care of over 95% of the problem. The rest is just mopping up that last ~5% that slip through the cracks via 3rd-party federation - such as public groups. But this does tend to reduce enragement engagement by total strangers dramatically - and if your product goals rely on ever-increasing MAUs, this probably isn't going to make it into the next sprint. Or the one after that.
