In my opinion, probably yes. If that information being made public is likely to make people feel uncomfortable, then its probably unethical.

I tend to block instances that block me, because I find it annoying to see an instance on the front page that I'm expected to look at but not allowed to interact with.

That actually does a pretty great job of getting rid of the explicit stuff. (Thanks for tagging your stuff, porn magazines!)
It does nothing for the just as NSFW anime magazines.

Unfortunately, there's a lot of NSFW that's not porn that I do want to see. Typically just some text.
On the other hand, spam bots seem to always mark their posts NSFW for some reason.
